School of Eligibility Application

Changing School of Eligibility: ASAA rule, Article 12, Section 2, A, 1c . states that:

A student attending a non-member charter school, alternative school, or program (including district correspondence) who wishes to change his/her School of Eligibility, will be ineligible for interscholastic competition at the new school of Eligibility for ninety (90) school days, including state approved in-services, after first attendance. (see Article 12, Section 9, 3).

A student enrolled in a program as specified above may request a change of School of Eligibility within a district by asking an administrator from  both the sending and receiving schools to complete this form and send it to ASAA. By sending this form to ASAA, both schools are acknowledging their agreement with this request. ASAA must receive this form and it must be approved by the executive director prior to any change of the School of Eligibility can occur.
